Keira Bell

Keira Bell is a British woman who was treated as a teenager at the Tavistock and Portman NHS Trust’s youth gender clinic in London. According to her own account, she was prescribed puberty blockers at sixteen, testosterone at seventeen, and underwent a double mastectomy at twenty. She later stopped taking the hormones and detransitioned.

The Lawsuit

Bell was the claimant in the case Bell v Tavistock, concerning whether minors can give valid consent to puberty blockers. On 1 December 2020, the High Court ruled that it is highly unlikely that children aged thirteen or younger are capable of doing so, and doubtful in the case of fourteen- and fifteen-year-olds. The Court of Appeal overturned that ruling on 17 September 2021, holding that judgments about capacity are for clinicians rather than the courts. In April 2022, the Supreme Court refused permission for a further appeal.

Her Own Story

The essay Keira Bell: My Story was published on 7 April 2021 on the platform Persuasion. In it, Bell describes growing up in Letchworth with a mother who had an alcohol problem and a distant father. She was a tomboy and became depressed and withdrawn at the onset of puberty. At fifteen, she was referred to the gender clinic. She describes the conversations that preceded the puberty blockers as superficial. In hindsight, she sees her gender dysphoria as an expression of deeper problems, such as depression, anxiety, and uncertainty about her sexual orientation. In her view, she needed psychological support rather than medical transition.

How the Case Began and What Followed

The proceedings were initiated in October 2019 by the mother of a fifteen-year-old patient, referred to as Mrs A, and by Sue Evans, a former nurse at the clinic. Bell replaced Evans in 2020. Following the High Court’s ruling, reports emerged that the clinic had stopped ongoing treatments. The organization Mermaids called the ruling potentially devastating, while Amnesty International and Liberty expressed concerns about its implications for consent and bodily autonomy. In a related case, AB v CD, the High Court ruled on 26 March 2021 that parents can consent to puberty blockers. In July 2022, the NHS decided to close the clinic; see the BBC report on this.
